The government assumes no financial responsibility for any costs associated with any response to this notice as incurred by prospective contractors. ****************************************************************************** Project Title: One (1) Non-PSC Primary Care Physician Tracking Number: IHS1524391 The Indian Health Service (IHS), Shiprock Service Unit (SSU), Four Corners Regional Health Center located at US Hwy 160 & Route 35, Teec Nos Pos, AZ 86514 is seeking capable, Native American Owned Small Business or Indian Economic Enterprise Small Business interested in providing the services to satisfy the requirement of a Contract Primary Care Physician for the Four Corners Regional Health Center, Urgent Care Department located in Teec Nos Pos, Arizona. The anticipated period of performance will be for 24 continuous months from the date of contract award. Contract POP shall include a Base plus three (3) Option Periods. Each period shall be 6 months each. The applicable North American Industry Classification System (NAICS) code assigned to this procurement is 621111 Offices of Physicians (except Mental Health Specialist) with a small business size standard of $16.0M. The government will evaluate market information to ascertain potential market capacity to provide supplies with those described in this notice. In accordance with the Buy Indian Act, 25 U.S.C. 47, the Indian Health Service shall give preference at all times, as far as practicable, to Indian economic enterprises. THE RESPONSES TO THIS SOURCES SOUGHT NOTICE/MARKET RESEARCH WILL BE USED TO DETERMINE THE ACQUISITION STRATEGY SUCH AS INDIAN-OWNED ENTERPRISE SET ASIDES, TOTAL SMALL BUSINESS SET ASIDE, ANY OTHER SOCIO-ECONOMIC SET ASIDE, OR UNRESTRICTED.
