List of clients over past 3 years, evidence that offeror operates and estabilished business with permanent address and license and permit by local law, Detail information of proposed room name, layout, lodging accomodation to include safety and emergency exit and any related information, and Insurance requirement (BPJS TK or BPJS TKBPU). The offeror shall submit the complete offer to the address indicated at Block 9, if mailed or hand delivered, of SF-1449, Solicitation/Contract/Order (page1).
