Project 658-25-103 Replace Xray Room 4 Site Prep Scope of Work Scope of Work: Contractor shall provide all labor, material, equipment, and qualified supervision for provision and installation for the task as described below. Concrete works: Provide Ground Penetrating Radar (GPR) services to establish required concrete penetrations. Removal old baseplate; patch and repair existing table location to include infill of concrete at baseplate. Metal works: Drill and anchor baseplates to include table and wall stand. Provide and install wall supports to include wall buckey for a complete and useable system. Provide modifications as needed to the existing Unistrut system. Electrical works: Provide, install, and revise, rework, and tie-in to new equipment all electrical components as needed for a complete and usable system: Existing warning light and emergency stop Conducts and electrical boxes Provide and install new loadside feeder. Rework existing 70A feeder to accommodate new 80A disconnect. Provide and install 480V/80A breaker at main distribution panel. Provide and install 480V/80A shunt trip breaker for disconnect. Provide power termination during equipment installation. Provide and install a total of seven (7) LED can lighting fixtures in the procedure room and control area to include LED dimmer switch. Provide and install IT and data drops as needed. Repair penetrations in firewalls with firestops as needed during execution of the work. Demolish existing light fixtures and HVAC diffusor. Patch and repair drywall ceiling. Patch and repair drywall walls to include repeat of lead disturbed by works cited above. Prepare and paint ceilings, walls, door and window frames, in the procedure room and control area. Provide and install welded seam flooring with integrated baseboard according to manufacturer s installation instructions. The current floor shall be removed and abated by Salem VAMC. Provide and install new HVAC diffusors in the procedure room and control area. Provide Stamped and Sealed Drawings indicating all works to include closeout documentation. Debris shall be placed in a dumpster and removed off-site for proper disposal. All the work shall be performed in accordance with VAMC Infection Control requirements.
