Quillen VAMC, Lamont Street and Veterans Way, Mtn. Home, TN 37684. SET-ASIDE: 100% Service-Disabled Veteran-Owned Small Business (SDVOSB) PROJECT MAGNITUDE: Between $10,000,000 and $20,000,000 NAICS: 237110 - Water and Sewer Line and Related Structures Construction SMALL BUSINESS SIZE STANDARD: $45 Million PROJECT OVERVIEW: STATEMENT OF WORK GENERAL INFORMATION Introduction - The James H. Quillen VA Medical Center (JHQVAMC) has a requirement to construction and replacement of approximately 24,000-lf of underground sanitary sewer piping of various size (3 to 24 ), approximately 44,000-lf of underground storm sewer piping of various size (2 to 42 ), manholes, curb drains, area drains, and basins. The work will include demolition, excavation, trenching, directional bore, piping, underground utility identification, backfill, compaction, paving, concrete, seeding and additional miscellaneous improvements and modifications as required to provide a complete and functional underground sanitary and storm sewer system. Scope of Work - The Contractor shall furnish all labor, supervision, materials and equipment to perform the following in construction and replacement of approximately 24,000-lf of underground sanitary sewer piping of various size (3 to 24 ), approximately 44,000-lf of underground storm sewer piping of various size (2 to 42 ), manholes, curb drains, area drains, and basins. The work will include demolition, excavation, trenching, directional bore, piping, underground utility identification, backfill, compaction, paving, concrete, seeding and additional miscellaneous improvements and modifications as required to provide a complete and functional underground sanitary and storm sewer system at the James H. Quillen VA Medical Center. Completely prepare site for building operations, including demolition and removal of existing structures, and furnish labor and materials and perform work as required by drawings and specifications. This work includes, but is not limited to, architectural plans, Civil, Structural, fire protection, plumbing, mechanical, electrical, data/communications, and physical security systems, within the Project Area. Work includes general construction associated with providing requirement to construction and replacement of approximately 24,000-lf of underground sanitary sewer piping of various size (3 to 24 ), approximately 44,000-lf of underground storm sewer piping of various size (2 to 42 ), manholes, curb drains, area drains, and basins on the James H. Quillen VA Medical Center. The area of work is located within a complex environment and will require ALL contractor personnel and subcontractors to comply with both security and infection control procedures outlined in the specifications. All selections of materials shall be in compliance with applicable standards and codes described in VA Program Guides & Design Specifications and shall be determined in coordination with Interior Designers at the VA facility. Work includes general construction, alterations, roads, walks, grading, drainage, demolition of existing piping structures, installation of sanitary and storm sewer piping and structures, and site restoration. The execution period for the project base is 485 calendar days. B. ALTERNATE A: BASE BID except delete all work included in Phase O. C. ALTERNATE B: BASE BID except delete ALTERNATE NO. A and sanitary sewer work in Phase C between Sanitary Sewer manhole SSMH37 and SSMH36. Both structures and all sanitary sewer pipes inclusive (~125 lf). D. ALTERNATE C: BASE BID except delete ALTERNATE A and ALTERNATE B and Sanitary Sewer Pipe H2 in Phase E. Upstream structure, SSMH40, inclusive, but not downstream structure (~87 lf). E. ALTERNATE D: BASE BID except delete ALTERNATE A, ALTERNATE B, ALTERNATE C, and all work in Phase D. F. IMPORTANT NOTICE: Apparent successful offerors must have a current VETS 4212 report on file with the Department of Labor, registered in SAM, and must apply for and receive verification from the Department of Veterans Affairs Center for Veterans Enterprise (CVE) of their SDVOSB status in accordance with 38 CFR Part 74 and VAAR 819.709 by submission of documentation of veteran status, ownership and control sufficient to establish appropriate status, offerors must be both visible and verified by the Department of Veteran Affairs CVE at the time of submission of bid. Failure to be both verified by Veteran Small Business Certification, (VetCert) and visible on Veteran Small Business Certification (sba.gov) at the time of bid submission will result in the bid being deemed non-responsive. All offerors are urged to contact the CVE and submit the aforementioned requirement documents to obtain CVE verification of their SDVOSB status if they have not already done so.
