Signed and dated Standard Form 30, if applicable, for any Amendments to the Solicitation. Completed schedule (pricing information) for all line items listed in the solicitation/amendments. Completed Offeror s Experience Form. Completed Subcontracting Information Form. Equipment list for vegetation maintenance and meeting ODF fire waiver requirements.
