DO NOT SUBMIT PRICING INFORMATION* The Contractor shall provide manufacturer-required preventive maintenance and corrective repair services for one EchoSens FibroScan MINI 430+ medical device. Services are limited strictly to medical equipment maintenance and do not include information technology services. The Contractor shall provide all labor, OEM parts, tools, test equipment, calibration resources, travel, and technical expertise necessary to maintain the device in accordance with EchoSens manufacturer specifications. *DO NOT SUBMIT A QUOTE OR ANY PRICING INFORMATION TO THIS NOTICE*

The Government hereby submits a notice of intent to award a sole source, firm fixed price purchase order to Fidelis Sustainability Distribution, LLC, under NAICS code 811210 Surgical instrument repair and maintenance services, by the Phoenix Area Office (PAO) Indian Health Services (IHS) under the authority of FAR 13.106-1(b)(2) (Soliciting from a Single Source). The requirement is for a for a contractor to provide Maintenance Service on EchoSens FibroScan MINI 430+ portable ultrasound system at the Phoenix Indian Medical Center (PIMC). This is NOT a request for quotes or proposals. Incumbent vendor, Fidelis Sustainability Distribution, LLC (SDVOSB), is the sole source vendor able to provide this service with a letter from the manufacturer stating they are the only distributor of this system. This will be a sole source procurement in accordance with RFO - FAR 6.103-1 Only one responsible source and no other supplies or services will satisfy agency requirements.
