The Louis Stokes Cleveland VA Medical Center is seeking sources that can provide Uniformed Security Services for the Parma Community Based Outpatient Clinic (CBOC), located at 8787 Brook Park Road, Parma, OH 44129. Response to this notice should include a capability statement, and three (3) customer references to which you have provided similar services. Responses should also include following: Company name: Address: Point of contact - name, email and phone number: Size of business pursuant to the following questions: (1) Is this service available under a federal supply schedule (GSA)? If yes, please provide the contract number. (2) Do you directly provide these services? If teaming please provide a copy of your teaming agreement. (4) If small, does your firm qualify as a small, emerging business, or small disadvantaged business?
