DRAFT STATEMENT OF WORK GENERAL: The Ann Arbor VA Medical Center (AAVA), located at 2215 Fuller Road, Ann Arbor, MI 48105, requires maintenance and cleaning of two (2) fish aquarium tanks, twice per month, located in the Community Living Center (CLC) and Acute Inpatient Mental Health (AIMH). SCOPE: The contractor shall provide the following services: Maintenance/cleaning site-visit once per month, on same date as one of the cleaning visits. Cleaning of two (2) 75-gallon fish aquarium tanks to include: Cleaning inside & outside of glass. Cleaning of gravel. Cleaning of filter system Provide any water tests/treatments as necessary. Adjusting plants, rocks, and general underwater scenery. Replacement of aquarium lights (burnt out, broken, malfunctioning). Replacement of filtering materials. After completion of services, Contractor shall clean the immediate area of any water, tools, sponges or any other items. Contractor shall provide fish food appropriate for fish species in auto feeders sufficient for feeding fish for one month at a time. Field Reports: Contractor shall provide a field service report documenting work performed on each visit. Failure to send a field service report may result in non-payment or delay in payment. Contractor shall collaborate with the Service POC/Contracting Officer s Representative (COR) regarding overall services provided. Plant and Fish replacements will be purchased on an as-needed basis and shall be purchased on a separate purchase order number. CONTRACTOR PERSONNEL SECURITY REQUIREMENTS: The Information Security Officer (ISO) requires that all contractors employees working on the contract shall be required to take the VA privacy training prior to starting work. GOVERNMENT RESPONSIBILITIES: Government will provide Contractor access to any rooms or equipment needed to perform the work. DAYS & HOURS OF OPERATION: Work shall be performed Monday through Friday, 7:00 am to 4:30 pm. Work shall not be performed on any federal holidays or any day deemed a federal holiday by the President of the United States.
