Telephone inquiries shall not be accepted. VA VISN2 intends to award a firm-fixed price indefinite delivery indefinite quantity (IDIQ) to a qualified firm who can meet the following requirement. VA Western New York Healthcare System, 3495 Bailey Avenue, Buffalo, NY 14215 requires transport services for patient samples that require testing at a local facility and for the transportation of blood products for transfusion between VA, local facilities and the American Red Cross Reference Laboratory in W. Henrieta, NY. The contractor shall provide the pick-up and delivery of blood specimens and blood products between the VA Western New York Healthcare System and local hospitals with Reference Tabs within 1.5 hours from the time the request for services is generated, 24 hours a day, 7 days a week, 365 days a year, including holidays. The contractor shall have the ability to supply occasional STAT delivery request entailing delivery times of less than 1 hour from the time of request. LOCATIONS Contractor shall supply service to destinations including, but not limited to, the following pickup/delivery addresses: American Red Cross Reference Lab 825 John Street West Henrietta, NY 14586 IMMCO 10 Earhart Drive, Suite 100 Williamsville, NY 14221 Buffalo General Hospital 100 High Street Buffalo, NY 14203 Erie County Medical Center 462 Grider Street Buffalo, NY 14215 Kaleida Health Center for Lab Medicine 115 Flint Road Amherst, NY 14421 Sisters of Charity Hospital 2157 Main Street Buffalo, NY 14214 Golisano Children s Hospital 818 Ellicott Street Buffalo, NY 14203 South Buffalo Mercy Hospital 565 Abbott Road Buffalo, NY 14220 VA Western NY Healthcare 3495 Bailey Avenue Buffalo, NY 14215 Contractor Responsibilities Personnel assigned by the Contractor to perform the services covered by this contract shall be licensed in a State, Territory, or Commonwealth of the United States or the District of Columbia. The contractor shall be Department of Transportation (DOT) certified. There may be hazards associated with specimen/supply transportation; therefore, the Contractor shall perform under this contract in a safe and appropriate manner and in accordance with all regulatory agencies, federal, state and local laws and regulations. Contractor shall complete OSHA approved training for transporting Bio-hazardous specimens. The Contractor shall have documented Blood Borne Pathogen Training for its employees. Each vehicle used in the performance of this contract shall be adequately stocked with at least one up-to-date complete SPILL KIT for a bio-hazardous cleanup in case of leakage or accidental discharge from controlled containers. The contractor shall ensure personnel performing services under this contract have the proper vehicle - license and training to perform the duties outlined herein. All vehicles and equipment are subject to inspection at any time during the life of this contract.
