**Update 9-2-2025** There was an error on the Attachment 10. QVL Application Form as of 6-5-2025 as it listed the "Construction Services on COMM facilities/equipment" category twice in error. A revised QVL Application Form has been uploaded, along with Amendment 3, correcting the duplicated entry to read as "Construction Services on NAVAIDS facilities/equipment (examples: MALSR, PAPI, VASI, REIL, ALS, VOR, etc.)". Both of the Construction Services on COMM facilities/equipment and Construction Services on NAVAID facilities/equipment categories should have been listed on the original QVL Application form. The corrected file is listed as Attachment 10. QVL Application Form as of 9-2-2025 and this is the file to be using for the QVL application form.
