The Department of Veteran Affairs, Veterans Health Administration, Network Contracting Office 10, Cincinnati OH 45219, intends to negotiate a sole source, Firm-Fixed Price Contract for a Hercules bed service agreement to Fidelis Sustainability Distribution, 204 West Spear Street, Suite 3319, Carson City, NV 89703 in accordance with FAR Subpart 13.2. This acquisition is for the Cincinnati VAMC, 3200 Vine St, Cincinnati, OH 45220. The Government intends to award the contract to Fidelis Sustainability Distribution, 204 West Spear Street, Suite 3319, Carson City, NV 89703. UEI: JSRRJ6NZFJD8. This notice satisfies the requirement at FAR 5.201(b)(1), requiring publication of the proposed contract action. This notice of intent is not a request for quote and no solicitation shall be made available. IAW FAR 5.207(c)(16)(ii) Contractors may submit a capabilities statement or quotation which shall be considered by the agency. The government shall not pay for any costs incurred by responses to this notice. This is a service contract in which the contractor shall provide a patient bed repositioner comprehensive service package for Cincinnati VA Medical Center (VAMC) in Ohio. This service includes unlimited technical support, service visit travel and labor, drive unit repair or placement, drive unit and Dream Air Mattress air controller, power cord placement, drive unit sheet strap replacement, mattress clip replacement, PM on drive unit, Dream Air Mattress air controller, mattress clips, Dream sheet assessment, Dream Gel or Dream Air replacement surface discount 15%, replacement mattress cover discount (installation included with purchase) 15%, Dream sheet discount 10%, Hercules transfer coverage “situational” included, necessity of on-site service visit to be at the discretion of the vendor after the initial service request to the needs of the government requiring facility at the Cincinnati VAMC by the contractor.
