SAM: SAM.gov | Home VETCERT https://veterans.certify.sba.gov/ Vet 4212 (Veteran owned small Business are reminded of Reporting Requirements) VETS-4212 Federal Contractor Reporting | U.S. Department of Labor (dol.gov) SCOPE OF WORK Furnish all labor, tools, supervision, equipment, materials, supplies, permits, and all other requirements necessary to complete the installation of spot repairs of seamless vinyl floors located in four Operating Rooms (ORs) at the VA Gulf Coast Veterans Health Care System, Biloxi, Mississippi. General Requirements & Scope of Work Contractor means any entity awarded this contract and any company working for the company awarded this contract. All work shall be performed with strict compliance to the Infection Control Risk Assessment, all applicable OSHA regulations, with safety of personnel being the utmost priority. A pre-bid site walkover of the areas covered in this service contract is highly recommended prior to submitting a bid. The condition of the sheet vinyl flooring at the ORs at Biloxi VA has been cited at recent Environment of Care rounding and Quality Management considers the flooring condition to be an infection control issue, and that repairs need to be made as soon as possible. Contractor performing the work must be competent and certified to work on and install seamless welded sheet vinyl flooring. A Competent Person is one who has documented and verifiable experience performing the installation of welded seamless floors and with meeting the infection control requirements for work in Operating Rooms at a VA Medical Center. Repair of seamless flooring is needed in each of the following Operating Rooms (OR): OR 1 requires repairs of approximately 163 square feet. Areas of approx. 6 x18 ,5 x5 , 6 x5 and 5 feet of cove base. OR 2 requires repairs of approximately 117 square feet. Areas of approx. 6 x15 , 3 x3 , 3 x3 ,3 x3 and 5 feet of cove base. OR 3 requires repairs of approximately 30 square feet. Area of approx. 5 x6 , and 3 feet of cove base. OR 4 requires repairs of approximately 40 square feet. Areas of approx. 6 x6 , 2 x2 , and 3 feet of cove base. Note: COR to direct contractor to section of cove base that needs this type of repair. Replacement sections of vinyl sheet flooring and width of seams are both to match existing. Verify thickness prior to install. Product of floor covering manufacturer welding rod in color shall match field color of sheet vinyl covering. Sheet vinyl flooring shall conform to ASTM F1913. Foam backed sheet flooring is not acceptable. Verify manufacturer is Mannington, Oyster White/15101. Pattern name BioSpec.
