VA is seeking sources that can provide the following services: 1. Guardianship services for Veterans who lack health care decision making capacity and do not have close family or designated health care agents 2. Identification of nationwide guardianship processes VA Medical Centers provide education for Veterans and families regarding advance health care planning and establishment of health care agent documentation. However, many Veterans lack the social resources of family or close friends to serve on their behalf in the vital health care agent role. VA seeks filing and disposition of guardianship proceedings services for Veterans who lack health care decision making capacity and do not have close family or designated health care agents. These services may be provided nationwide (potentially including all 50 states and US territories) or by region. To the extent services are unavailable nationwide, VA is currently seeking services to cover the catchment areas of VA facilities located in Florida, Texas, and/or California. The requirements and process for initiating guardianship proceedings vary nationwide. VA seeks services for identification of state and local requirements for applying for guardianship of the person, guardianship of the property (estate), proxy for healthcare decision-making, or the state or local equivalent, relevant to all VA healthcare locations nationwide. Vendors must be registered in the System for Award Management (SAM.gov). Interested vendors should answer the following questions in 2 pages or less. 1. What guardianship services does your organization provide? 2. Where does your organization provide services? Please list the states and/or U.S. territories where your organization provides guardianship services. 3. What is your organization’s rough estimate of costs for guardianship services? If your organization has the potential capacity to perform these contract services, please provide the following information: 1) Organization name, address, email address, website address, telephone number, and size and type of ownership for the organization; applicable NAICS codes, and 2) Tailored capability statements addressing the particulars of this effort as outlined above, with appropriate documentation supporting claims of organizational and staff capability. If significant subcontracting or teaming is anticipated to deliver technical capability, address the administrative and management structure of such arrangements. The government will evaluate market information to ascertain potential market capacity to: 1) provide services consistent in scope and scale with those described in this notice and otherwise anticipated; 2) secure and apply the full range of corporate financial, human capital, and technical resources required to successfully perform similar requirements; 3) implement a successful project management plan that includes compliance with program schedules, cost containment, meeting/tracking performance, hiring/retention of key personnel, and risk mitigation; and 4) provide services under a performance-based service acquisition contract. Based on the responses to this sources sought notice/market research, this requirement may be set aside for small businesses or procured through full and open competition, and multiple awards may be made. The government will not provide feedback or evaluations to companies regarding their submissions in response to this notice. Submission Instructions: Interested parties who consider themselves qualified to perform the above-listed services are invited to submit a response to this sources sought notice by 12:00pm (noon) EST on 22 September 2025.
