The NAICS code for this requirement is 621210 - Offices of Dentists. The Product Service Code (PSC) designation is Q503 - Medical- Dentistry. The SBA Size Standard is $9M. Evaluation of the quote will be based on a Trade Off process; award will be made to the vendor whose response represents the Best Value to the government. **NOTE** Please see the FAR clause 52.212-2 Evaluation Commercial Products and Commercial Services. Offerors must complete and submit the SF1449 Solicitation form. Offerors must submit the provided Dental Treatment Pricing Sheet for either the Reno/Carson City Area or Las Vegas Area or both. Offerors must also specify the name and location of the Dental Office where services will be performed. Past performance will be requested if the Government considers it necessary during evaluation of offers. Amendment 0001. The purpose of this amendment is to answer questions from interested vendors. In providing answers to the questions, treatment service lines were added to the price list and the Performance Work Statement is revised accordingly. Revisions to paragraph 5.6.16 (removed entirely) along with adding required treatment services under appendix A are annotated in red. The date and time for quotes due IS extended to Monday, September 22, 2025 at 8:30 p.m. PDT.
