AMENDMENT # 2BTD AUTOCLAVES AND GLASSWASHER PM&R W911S625QA005

Maintenance and service for autoclaves and glass washers (PM&R) require Offerors/Contractors to show proof of liability insurance and to demonstrate proficiency, relevant experience, credentials, and training certificates from the equipment manufacturers, as well as confirmation of Authorized Service Agency (ASA) status for the listed equipment. The evaluation factors now include these credentials and manufacturer training certificates, with proficiency and relevant experience addressed in the Performance Work Statement sections 2.f and 3.h. The Insurance clause 52.228-5 is incorporated by reference, and revised documents are attached for review. Respondents must submit one signed copy of all amendments with their quotation.

Amendment #2 adds explicit proof requirements and updates evaluation criteria to require insurance, credentials, manufacturer training certificates, and ASA confirmation; it clarifies that proficiency and experience are evaluated per the PWS. Revised documents are attached, and respondents must sign and submit with their quotation, along with all amendments.
