Verbal responses are not acceptable and phone calls will not be accepted.

Interested parties may express interest by submitting a capabilities statement demonstrating the ability to provide the WAIS-5 Complete Kits. Responses must address: your organization’s socio-economic category; whether you manufacture the supplies (or identify the manufacturer and provide evidence of authorized distribution); a completed FAR 52.225-2(b) country-of-origin certificate (USA if a US domestic end product); whether you meet the requirements of 13 CFR 121.406(b)(1)(i)-(iii); and whether subcontracting will be contemplated and, if so, the percentage and tasks. Note potential applicability of VAAR 852.219-76 and 852.212-71 if the contract is set aside for Veteran-Owned Small Businesses. This is not a request for competitive quotes.
