The North American Industry Classification System number is 325413 with a size standard of 1,250. 5. Place of Performance: Overton Brooks VA Medical Center 510 E. Stoner Avenue, Basement BE 30 Shreveport LA, 71101 6. Period of Performance: Period of Performance: The period of performance shall be for one (1) Base Year of 12 months and four (4) 12-month optional performance periods. The Period of Performance reads as follows: Base Year September 25th, 2025 thru September 24th, 2026 Option 1 September 25th, 2025 thru September 24th, 2027 Option 2 September 25th, 2027 thru September 24th, 2028 Option 3 September 25th, 2028 thru September 24th, 2029 Option 4 September 25th, 2029 thru September 24th, 2030 7. RFQ 36C25625Q1470 Question and Answer Reponses The purpose of this amendment is to: Clarify that this requirement is LPTA for brand name or equal to products. Clarify that all CLIN 0001-4026 are all brand name or equal to products and the OEM for the CLINs listed is Werfen. Address the questions submitted in response to the solicitation question deadline of 3pm 09/09/2025. See below Vendor 1: Question: Would it be possible to provide the manufacturer for the items on the subject RFQ as we can likely assist however, I am not able to find the source? Government Response: The OEM for the items listed in the CLIN section is Werfen. Vendor 2: Question: *Page 72 of 128 of the solicitation, Anti-M reagent appears twice, is this a duplicate or is it required? Government Response: This was an accidental duplication.
