Transaction Number: 678-25-3-071-0122 2.Brief Description of Supplies/ Services required and the intended use/Estimated Amount: This acquisition is in support of the Tucson VA Healthcare System, Ophthalmology department has a need for several ophthalmic floor stands with instrument consoles. These items include chair controls to adjust to patient heights as well as add-ons to accommodate wheelchairs. Installation will be required for these items. Estimated amount: $119,139.46 3. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used: This procurement does not exceed the five-million-dollar limit for utilizing the authority at VAAR 819.7007. Corps Medical Supply is a verified SDVOSB that has a proven record of providing the needed supplies at a fair and reasonable price. 4.Description of market research conducted and results or statement why it was not conducted: A search of the VA National Strategic Source Listing (SAC) national mandatory contracts did not reveal BPA s or IDIQs that contain the required equipment. Additional Market research was also conducted to verify that Corps Medical Supply is a Small Business certified SDVOSB vendor, that they are registered in the Systems for Award Management (SAM), and they do not appear on the Excluded Parties List (EPLS). 5.Contracting Officer's Certification: Purchase is approved in accordance with FAR13.106-1(b) and VAAR 806.302-5-70. I certify that the foregoing justification is accurate and complete to the best of my knowledge and belief.
