This is a SOURCES SOUGHT NOTICE (SSN) and REQUEST FOR INFORMATION (RFI) only. Responses to this notice will be used for information and planning purposes as part of market research. No quotes are being requested or accepted at this time with this notice. Responders are solely responsible for all expenses associated with responding to this notice. The Department of Veterans Affairs, Altoona VA Medical Center located at 2907 Pleasant Valley Blvd, Altoona , PA 16602 has a requirement for bariatric patient chairs delivered FOB Destination to the Altoona VAMC. The North American Industry Classification System (NAICS) code being considered for this procurement is 337127 Institutional Furniture Manufacturing. The Small Business Size Standard is 500 employees. Information is being requested from industry to determine the following: Can your business provide bariatric chairs which meet the salient characteristics of each chair type listed in Section 3 of the Statement of Need? VELA brand chairs have been identified as acceptable, please identify any alternative chairs by including the manufacturer, part number, and sufficient product information to demonstrate compliance with the Statement of Need. Interested and capable sources are requested to respond to this notice with the following information: Company name SAM Unique Entity Identifier (UEI) Company s address Point of Contact information (i.e. title, phone number and email address) Company s capability to meet this requirement and any pertinent information which demonstrates the company s ability to meet the above requirements. GSA Federal Supply Schedule contract number or other applicable contract vehicle contract number, if within scope of this effort (if applicable). Company s type of business (small or large) and socioeconomic status whether Service Disable Veteran Owned, Veteran Owned, Hubzone, 8(a), Women Owned, Small Disadvantaged, etc. If applicable, Service-Disabled Veteran Owned Small Businesses (SDVOSB) and Veteran Owned Small Businesses (VOSB) must be verified in Veteran Small Business Certification (sba.gov). Responses from SDVOSB or VOSB concerns should provide enough information to determine if the vendor can comply with the limitations on subcontracting as defined in VAAR 852.219-76 and all applicable CFR references where applicable.
